Reporting Concerns
To maintain an inclusive and safe learning environment, the university has tools in place to respond to incidents that impact our students, employees, or guests of the university.
Below are links for reporting general concerns and specific incidents.
What happens to reports?
When a report is submitted, it is reviewed by designated personnel who evaluate and respond, as appropriate, to each incident. Each response will be in accordance with university, UW System, and state policies or statutes.
Submitting an incident form is not the same as filing a police report or pressing charges. To submit a police report, contact University Police.
Submitting an incident report does not always result in disciplinary action by the university.
You can submit a report for the following types of incidents:
Bias incidents are acts motivated by bigotry and/or hate based on the victim’s actual or perceived race, ethnicity, nationality, gender, sexual orientation, gender identity/expression, religion or disability. Hate crimes are those acts that violate criminal law, such as violence or vandalism motivated by bias against a member of a protected group.
If you believe you have witnessed or are a victim of a bias incident or hate crime, no matter how small the incident may seem, you can report that here. This report is sent to the Associate Dean of Students for Equity, Diversity and Inclusion for review. The Associate Dean for EDI chairs the Bias Response Team.
The mission of the Behavior Intervention Team (BIT) is to promote a safe and healthy learning environment at UW-Stout by detecting, assessing and providing early intervention to students whose behavior raises concern for serious health and safety risks to themselves or other members of the campus community.
If you have observed behavior that causes concern for someone’s well-being, is disruptive and/or threatening, or poses potential for harm to self or others, you can report that here. This report is sent to the Dean of Students office for review.
The state of Wisconsin mandates any UW System employee to report incidents of child abuse or neglect to appropriate officials or local law enforcement.
If you observe an incident or threat of child abuse or neglect or learn of an incident or threat of child abuse or neglect, no matter how small the incident may seem, you can report that here. This report goes to University Police Chief, Jason Spetz.
UW-Stout is committed to preventing all forms of sexual misconduct, including sexual assault, dating and domestic violence, stalking, and sexual harassment. All are university policy violations if they involve students, faculty, or staff, whether they happen on campus or off campus.
If you believe you have witnessed or are a victim of sexual misconduct, no matter how small the incident may seem, you can report that here. This report is sent to the Title IX Coordinator for review.
If none of the above categories match the incident you witnessed or are victim of, file a general incident report here. Provide as many details as you feel comfortable sharing. The report will be received by the Dean of Students Office and will be appropriately routed upon review.
